Marshfield Clinic Sports Medicine provides pre-participation sports fitness screenings required by the Wisconsin Interscholastic Athletic Association (WIAA) for students in organized athletics. Screenings are offered at select locations—especially in late spring and summer—so you can check this off your list before the season starts.
What this screening does (and doesn’t) do
- Confirms readiness for physical activity as required by the WIAA. It’s general in nature and not a substitute for routine health care with your regular clinician.
- Can be completed during a routine well-child visit with your child’s pediatrician or primary care provider, if you prefer a one-and-done appointment.

Who can sign your WIAA form
A licensed physician, surgeon, nurse practitioner, or physician assistant may perform the screening and sign the official WIAA form.
How long are WIAA forms valid
- WIAA forms are valid for two calendar years from the screening date.
- In the second year, a parent or adult athlete must sign an “alternate year” form.
- To be valid for the next school year, complete the screening after April 1.
Quick checklist (bring & wear)
Bring
- Completed WIAA physical exam form and health history form signed by a parent or legal guardian (before check-in).
Wear
- Female athletes: shorts + one-piece swimsuit + T-shirt.
- Male athletes: shorts + T-shirt.
- All athletes: athletic shoes (no sandals or boots).
Prep
- Avoid caffeinated beverages for at least 4 hours before your appointment.
- Avoid heavy exercise for at least 2 hours beforehand.
